About LillyCare

WHO WE ARE: LillyCare a Pediatric & Family Medical - Dental Center dedicated to providing comprehensive, highquality
preventive care for all ages. Our services for the working poor and uninsured
include dental and mental health care. We also house an on-site pharmacy for our
uninsured patients.

Nearly 90% of LillyCare's patients are below 100% of the poverty level. About 88% of
our patients are Latino, and 34% are age12 or younger. Approximately 68% are
female. In 2013, we want to serve more than 3,520 patients at our four school-based
teen clinics in local schools.

WHAT WE DO: Established in 2013, LillyCare is looking to open in July 2013 caring
for the underserved communities in the downtown and South Los Angeles areas.

WHOM WE SERVE: Our patients are the working poor, the uninsured, and the
underserved communities central Dallas. In 2010, 67% of our more than 86,000
patients lived within the 75219 zip code listed 80% resided (Source: UDS).

WHERE WE'RE LOCATED: The LillyCare home is located at 4333 Maple Ave /
WyCliff Dallas, Texas 75219. Our new home sits within a unique building graced with
murals, with children theme, cheery wall coverings and furniture, and modern medical
suites.

WHEN WE'RE OPEN: Clinic hours are 8 AM - 9:00 PM, Monday through Saturday.
The Pediatrics Clinic opens at 7:30 AM. Most clinics have evening hours until 6:30 or
