The Ryan-NENA Community Health Center strives to provide affordable, quality medical services at a fraction of the cost. Patients can expect to be treated with dignity, confidentiality and respect.
Accepts Walk in patients.
To register as a new patient, we ask that you bring:
- Photo identification, if available
- If you have insurance, your insurance card and any applicable co-payment
- If you do not have insurance, please bring proof of income (e.g., pay stubs or tax return) and proof of residence (e.g., lease, utility bills, other bills addressed to you) to qualify for our sliding fee scale
To ensure the best possible continuity of care, please bring the following documents if you have them:
- A copy of your medical record
- A list of your current medications and your medication bottles; including prescription medications, over-the counter medications, herbal supplements, vitamins, and eye drops
- For children, a copy of their birth certificate and immunization record
For those families without insurance, payment is based upon a family's size and income. If you pay on the day of your visit, you will receive a discounted rate. Laboratory work, general x-rays and EKGs are included in the fee for each visit. The Ryan Center offers a low cost pharmacy plan for uninsured patients.
Payment Assistance
The Ryan Health Network's Patient Accounts Department can also assist you in applying for:
- Medicaid
- Child Health Plus (CHP)
- Family Health Plus (FHP)
- ADAP Plus
- Family Planning Benefits Program
- Cancer Screening Services
- Presumptive Eligibility for Women
Direction: Take the F subway line to Second Avenue or Delancey Street; or the J,M or Z subway lines to Essex Street. The site is within blocks of the M14, M21 and M29 bus lines.

I would like to know if you have health insurance representatives..I have my grandson n he works m looking for low income insurance..he's 27 years old..
